About Stokes Case Management Ltd.
Stokes Case Management Ltd. was established in 2015 by its Clinical Director, Rhiannon Stokes, in response to the growing need for a new wave of case management and rehabilitation services for individuals and professionals in Wales and the UK.
It is our mission to cater, not only for the specific case management, rehabilitation and medicolegal needs of disabled and vulnerable individuals, but also for the increasingly complex support services that are necessary to support these individuals.

Please note, we are using the self-hosted version of MachForm, which means your data is not routed through or stored on MachForm cloud servers. The data we input via the Self-hosted MachForm forms are sent to our own MySQL database, via a secure (SSL) connection.
Information we store on our MachForm system
We store notes and information about your progress and rehabilitation with your support team. This is logged securely via MachForm by fully trained team members.
Access to MachForm data
Your MachForm data will only be accessible by approved and fully trained members of our team who are aware of and will comply with GDPR.

We use notes and information stored on our MachForm system to log your progress, your needs or requests, and any information that is relevant to your ongoing rehabilitation.
Sharing MachForm data
We may have to share your MachForm data with third parties, such as other health professionals that you are working with as part of your rehabilitation.
Consent to store your data on our MachForm system
We will explain our processes carefully to all new clients. Consent forms must be signed by clients before their personal data can be stored on our MachForm system. We will periodically assess and update our consent forms as required to ensure that we are fully GDPR compliant and that you are aware of our current processes and your rights relating to them.
Storing MachForm data
All data submitted via our Machform system is sent via a secure (SSL) connection, through to a secure MySQL database. Our hosting and database service, is provided by Cloud Above (https://cloudabove.com/), managed by Madamadari Development and Design.
