Sian is a Case Manager at Stokes Case Management, as well as a specialist neuro occupational therapist who has a breadth of experience working within acute rehabilitation and community settings.
Since qualifying, Sian has worked with a wide range of clients with various medical conditions such as amputees, spinal injuries, orthopaedic injuries, stroke, traumatic brain injuries and degenerative conditions. Sian also has a well-grounded understanding of mental health conditions through her background in psychology and working with those who have primary or secondary mental health diagnoses.
Sian is experienced in complex discharge planning, community interventions and establishing care packages for clients. Sian has often worked with clients from the beginning of their diagnosis through to establishing their independent living. She is highly skilled in risk assessment, major and minor adaptations, intervention plans, and therapeutic intervention. Sian has essential leadership skills that are required to effectively and efficiently achieve outcomes.
Sian is incredibly passionate about advocating for clients to ensure that they are supported in the best possible way to maximise their full rehabilitation potential and quality of life. She has excellent communication skills and works holistically to manage multidisciplinary input. Sian prides herself on building a very good rapport with clients who can put their trust in her to maintain a client-centred focus throughout.
