Emma is a compassionate and dedicated Case Manager, committed to empowering individuals to overcome barriers, access opportunities, and fulfil their potential.
Highly organised and client-focused, she has experience managing complex cases, coordinating MDTs, overseeing directly recruited support teams, collaborating with external agencies, and supporting both settled and unsettled cases. Skilled in navigating services, providing clear guidance, and working closely with families, Emma ensures coordinated, effective, and compassionate care. Through her extensive experience in the health and social care sector, she has developed expertise supporting individuals with a wide range of needs, including physical and learning disabilities, mental health conditions, dementia, and ASD.
Emma is committed to ongoing professional development and regularly attends conferences, workshops, and training events, ensuring she remains up to date with the latest evidence-based knowledge and best practice. Engaging in these opportunities also allows her to build and maintain a strong professional network, which she can draw upon to enhance the support and outcomes she provides for her clients.
She excels at fostering collaborative working environments and possesses strong mediation skills. As a former safeguarding lead, she has worked closely with multi-agency teams, contributing to CAF panels and Child Protection Plans.
As a Learning Mentor in a mainstream school, Emma took a holistic approach to supporting children in overcoming barriers to learning. The role involved regular collaboration with medical professionals to ensure tailored support and effective interventions for children with complex needs. In addition, her position as First Aid Coordinator provided valuable experience in managing care plans, completing risk assessments, and overseeing medication administration.
Emma has a proven track record of building strong relationships and developing tailored support plans to help individuals and families achieve meaningful SMART outcomes. She has both professional and personal experience supporting children with SEN and navigating the EHCP process. Committed to person-centred care, Emma empowers others to have a voice. Her experience includes advocating for parents with learning disabilities, safeguarding the rights of young people and adults with learning disabilities detained at police stations, facilitating peer-education projects, managing youth-led teams, and planning, organising, and supporting young people to attend international youth conferences.
Her leadership extends beyond direct support, having successfully managed and trained individuals and teams across various settings. Additionally, Emma’s experience as a property manager and director of a specialised construction company has equipped her with valuable business skills, including HR, health and safety, risk assessment, budgeting, and payroll.
